Mechanism Of Action
Sotalol blocks both ß1 and ß2 receptors.
Indications
sotalol is used in the management of ventricular and supra ventricular arrhythmias. It was also formerly used in the management of angina pectoris, hypertension and myocardial infarction (MI). But, because of its proarrhythmic effects such as torsade de pointes, New Ventricular tachy cardia (VT) or new ventricular Fibrillation (VF), it is no longer recommended for these indications. Its use in arrhythmias is restricted to severe disturbances. It should not be used in patients with asymptomatic ventricular arrhythmias.
Side Adverse Effects
chest pain, palpitation, vomiting, arrhythmias such as: Av block, brady cardia, PVC, VT, Torsade de points and prolonged QT interval.
Contraindications
Sinus bradycardia, AV block, cardiogenic shock, Hypersensitivity to drug, CHF, prolonged QT interval (Congenital or aquired ) , Bronchospasm and asthma.
Warnings Precautions
It may cause to new arrythmiaes or aggriated arrhythmias, so patients should be monitored. There is caution in about taking sotalol in patients with sick sinus syndrome (SSS), Hypokalemia , hypomegnesmia, patients with systolic Blood pressure lower than 90mmHg and peripheral arterial diseases.

FDA pregnancy category B.
Breast Feeding
Because of the potential for a serious reaction in nursing infants from beta adrenergic blockade a decision should be made about whether to discontinue nursing or to discontinue sotalol.
Drug Interactions
Sotalol should not be used with other drugs that prolong the QT interval due to the increased risk of precipitating ventricular arrhythmias. Thus sotalol should not be given with antiarrhythmics that prolong the QT interval (such as amiodarone, disopyramide, procainamide, quinidine), phenothiazine, antipsychotics, tricyclic anti depressants, terfenadine, astemizole, cisapride, erythromycin, halofantrine, pentamidine.
Administration And Dosage
To control acute arrhythmias, a dose of 20 to 120 mg is given intravenously over 10 minutes. The total daily dose should not be exceeded 640mg. The dose should be reduced in patients with renal impairment.
