Pharmacologic Category cholinesterase inhibitor, antidementia. Mechanism Of Action Rivastigmine inhibit the enzymic hysrolysis of acetylzholine by acetylcholinesteras and other colinesterases. There by prolonging and enhancing its actions in the body. Rivastigmine is centeally acting and reversible. Indications * mild and moderate dementia in alzheimer's disease and Parkinson (1.5mg per oral two times daily with food. There after, the dose may be increased according to response by increments of 1.5mg twice daily at interval of at least 2 weeks to a maximum dose of 6mg twice daily. Side Adverse Effects Neasu, vomiting, anorexia, fatigue, dizziness, dyspepsia, abdominal pain, headache, somnolence, insomnia, muscle cramps, depression, constipation, Rhinitis, sweating, extrapyramidal symptoms, psychiatric disturbance, agitation, hallucination, confusion, tremor. Contraindications Hypersensivity to rivastigmine and other carbamats. Warnings Precautions it should be used with care in patients with impaired liver function. Pregnancy FDA pregnancy category B. . Breast Feeding It is not known whether Rivastigmine distributed in milk, thus its consumption may be effect on intants. Drug Interactions Drugs with neuromuscular blocking activity such as the aminoglycosides, clindamycin, colistin, cyclopropane, and the halogenated inhalational anaesthetics, may antagonise the effects of rivastigmine. A number of drugs, including quinine, chlroquine, hydroxychloroquine, qunidine, procainamide, propafenone, lithium and beta blockers that have the effectiveness of treatment with parasympethomimetics. Anticolin esterases such as rivastigmine can inhibit the metabolism of suxamethonium and enhance and prolong its action.
